New Patient Registrationregister_now

If you live in the practice catchment area you are eligible to join our list. When you attend to register you need to bring photographic identification with you. You will be asked to complete a registration form and health questionnaire for each person and will be offered a new patient appointment with the practice nurse or healthcare assistant. At your new patient appointment the practice nurse or health care assistant will review your health questionnaire and perform some simple health checks.

If you are newly registering and need medication on a regular basis, you will need to make a GP appointment in advance of wanting it issued.

Patients register with the practice and not with a named doctor. You may choose to receive services from the doctor of your choice unless he or she does not routinely perform the service in question within the practice.

Medical treatment is available from the date of registration. Please contact reception for further information.

You may print off a registration form, fill it out and hand it in to the surgery before making an appointment.
